How to refer a family for home visiting services:

Step 1

Make A Referral

Anyone may refer a family to our program by contacting us directly or completing the referral form above. If reaching out directly, please include basic information on family’s situation and ways to contact and send an email to referrals@familynetworknwa.com.

A family is considered eligible if:

  1. Mom is prenatal    

  2. Baby is newborn and still within the first couple weeks of life.

  3. At least one of the other eligibility factors is met. These can include but are not limited to: 250% of the poverty line, teen pregnancy, transient/homelessness, substance abuse history, child abuse history, low educational attainment, caregiver disability, child developmental delay.

    If in doubt, just send the referral and we’ll make the screen!


Step 2

Schedule a Visit 

Family Network staff will reach out to collect further info, explain more about program, and schedule an in-person visit in the home within 1 week of receiving the referral. 


Step 3

Join the family

After the first visit in the home, families either accept or decline services. If they accept, enrollment forms are filled out and a schedule is made to visit family weekly.
